What is CHDP?
The Child Health and Disability Prevention (CHDP) Program is a statewide program, whose goal is to help keep children and teenagers healthy and prevent disabilities, by finding and treating health problems early.
The CHDP Program
Provides preventive health exams and immunizations to children and teens from low to moderate income families, from birth through age 18 years and for Medi-Cal eligible children from birth through 20 years of age.
Educates Orange County communities about the importance of regular preventive health care for all children, and the value of having a medical home.
Informs Orange County families about the physical health exam requirements for first graders.

Families can get a referral for up to three CHDP providers in their area by calling:

Phone staff can provide more information and help with scheduling an appointment or finding a bus route to get there.
The Health Referral Line can also provide information about other health resources available to Orange County children.
Who is Eligible for CHDP Check-ups?
Resident of California
Younger than 19 years of age
Low to moderate income families
Head Start or State Preschool children or
Medi-Cal or Healthy Families eligible from birth through 20 years of age
CHDP Services
All services provided by the CHDP Program are free
Services are available from a network of over 300 doctors and community clinics
The CHDP Program provides regular periodic health check-ups (also known as a health assessment, screening, and physical exam) to children and teenagers
The doctor visits are more frequent for babies and young children, less frequent for teenagers
Children may also receive additional physical exams needed for school, summer camp, or sports
CHDP services meet the requirement for enrolling in WIC, Head Start, School sports, or camp
For certain conditions found in the CHDP check-up, children and teenagers may receive follow-up care through the CHDP Treatment Unit (CTU)
The Health Check-up
Like all good pediatric health check-ups, this includes:
Health history and an unclothed head-to-toe physical exam
Dental, nutrition and developmental assessments
Immunizations
Vision and hearing screenings for children 3 years and older
